Occupational Therapy Invoice Template Canada: Example and Checklist
A Canadian occupational therapy invoice needs to tell the payer who received the service, who delivered it, what was supplied and how the amount was calculated. It should also be easy to reconcile with the appointment record and any payment received. An attractive PDF cannot fix a wrong client name, service date or tax decision.
Use the copyable invoice structure below as a starting point for private-practice administration. It is an on-page example, not an official insurer form. Requirements differ by province, profession and payer; Ontario-specific guidance is identified rather than presented as a Canada-wide rule.

Separate the provider, client and payer
These can be three different parties. The client is the person who received the service. A parent, employer or insurer may be the payer. Record both roles accurately rather than replacing the client’s identity with whoever is paying.
COTO’s billing guidance explains Ontario expectations for financial records, provider identification, services, dates, fees and payment information. It also advises checking a third-party payer’s requirements. Outside Ontario, consult your provincial regulator and relevant funding agreement.
Copyable Canadian OT invoice example
Every name and amount in this example is fictional. The CAD 150 hourly rate is for arithmetic only, not a recommended fee. This example assumes the supplied service has been confirmed as GST/HST-exempt; do not copy that treatment to an assessment or report without checking its purpose and your own circumstances.
| Field | Example entry |
|---|---|
| Provider | Example OT Practice; treating OT name, designation and applicable college number |
| Business contact | Practice billing address, telephone and email |
| Invoice reference | OT-2026-001; issued 22 October 2026; currency CAD |
| Client and payer | Client: Example Client A; payer: client, or separately identified third party |
| Service | 20 October 2026; occupational therapy treatment session; actual provider identified |
| Calculation | 60 minutes ÷ 60 × CAD 150 per hour = CAD 150.00 |
| GST/HST | Exempt in this fictional scenario only; tax charged CAD 0.00 |
| Invoice total | CAD 150.00 |
| Payment received | CAD 50.00 on 21 October 2026 by the recorded payment method |
| Balance due | CAD 100.00; agreed due date and payment instructions |
Invoice, receipt and insurer form are different documents
An invoice records the charge and balance. A receipt records a payment that has actually been received. If only part has been paid, show the payment and remaining balance without marking the full invoice paid. Keep the invoice reference consistent so the documents can be matched later.
A payer may require its own submission form, authorisation reference or billing route. A general invoice does not replace those requirements and cannot promise reimbursement. Confirm what is needed before spending time recreating the invoice in the wrong format.
For example, an Ontario attendant-care assessment has its own purpose and supporting documentation. The Form 1 guide explains that context; the assessment form is not a general treatment invoice.
Decide GST/HST before copying a tax line
The CRA’s P-256 policy distinguishes qualifying health-care purposes from other assessment and report purposes. Your job title alone does not settle the tax treatment of every supply. Do not label a service exempt merely because an OT performed it.
The CRA invoice and rate guidance explains place-of-supply rates and information required for taxable sales. Exempt and zero-rated are not interchangeable labels. Confirm classification, registration obligations and the applicable rate with an accountant when uncertain.
For the detailed distinction between treatment, insurance assessments and medico-legal work, read the existing GST/HST guide for Canadian therapists. Keep separate services visible on separate lines instead of using one vague “therapy services” total.
A pre-send check for a private OT practice
Review the invoice against the actual service record, not a previous client’s invoice. Save a version you can identify later and use the approved process for corrections. Follow your professional and organisational record-keeping requirements rather than deleting a financial record when the balance reaches zero.
Only include clinical detail that is appropriate and required for the billing purpose. A finance document is not the place to paste a full assessment narrative. If the payer asks for more information, check the request and disclosure basis before sending it.
- Correct client and payer, without copying another person’s identifiers.
- Actual provider, service date, description, duration or quantity and agreed rate.
- Arithmetic checked: line totals, any applicable tax, payments and remaining balance.
- Tax wording appropriate to this service, not inherited from the example.
- Payer-specific reference or submission requirements confirmed.
- Correct recipient and payment instructions, with a retained financial record.

Frequently asked questions
Is this a free downloadable Canadian invoice file?
This page provides a free, copyable invoice structure and fictional example, not a downloadable insurer form. Carearoo also provides free Canadian invoicing; check the Canada page for current availability.
Do all Canadian OT invoices need the same fields?
No single example covers every province, payer and purpose. Use your provincial regulator’s requirements, any payer instructions and applicable tax rules. The COTO guidance linked here is for Ontario.
Should an OT invoice always show GST/HST?
No. The treatment depends on the nature and purpose of the supply and relevant tax rules. Confirm the classification and your registration position rather than copying an exempt or taxable line from a sample.
Can I generate Canadian assessment reports in Carearoo now?
Canadian invoicing is live. Canadian report drafting is not yet available; see the country page for planned types and early-access information.
